What documentation is sufficient to prove my Citizenship?

There are 2 ways you can complete the Citizenship Verification requirement, submitting documentation online via the StudentForms portal or submitting your documentation in-person at the Office of Scholarships and Financial Aid.

1. Submit online via the StudentForms portal.

You will be asked to complete to:
1. Complete the Affidavit Web Form
2. Submit a copy of a governement issued ID
3. Submit a copy of one of the documents listed below

  • Birth Certificate
  • U.S. Passport or Passport Card (signed)*
  • U.S. Travel Document
  • Certificate of Naturalization (signed)
  • Certificate of Citizenship (signed)
  • Certificate of Birth Abroad

*Please note that you can no use your passport twice for your proof of citizenship and your government issued Photo ID. Unfortunatly we can not accept military IDs as documentation. A helpful guide for this process can be found here.

2. Submit your documentation in-person at the Office of Scholarships and Financial Aid
Submit a copy of one of the documents listed below to one of our OSFA representatives:

  • Birth Certificate
  • U.S. Passport or Passport Card (signed)
  • U.S. Travel Document
  • Certificate of Naturalization (signed)
  • Certificate of Citizenship (signed) Certificate of Birth Abroad
