Filling out your FAFSA is the first step to receiving financial aid for college, but in some cases additional information may be needed from you. There are a variety of forms that cover various circumstances.

Many forms are accessible online, only through your Verify My FAFSA account and need to be assigned to you by our office. It is important that you continue to monitor your To Do List and your CatMail for these forms. Other times, you may be required to download a form listed below and submit it directly to our office. For more details on how to download and submit your form, see instructions below.

Forms and/or documents can be submitted online via our Ask Aid Portal at https://financialaid.arizona.edu/contact/email per the instructions below. Third party organizations can upload documents on behalf of the student. In order to do so, you will need to provide the student’s full name, University of Arizona email, and (8 digit) University of Arizona Student ID number.
The Office of Scholarships and Financial Aid is located in the Administration Building, Room 208.

2020-2021 Parent PLUS Loan Special Circumstances Form
The Parent PLUS Loan Special Circumstances form is for students whose parents are unable to apply for the Federal Direct Parent PLUS Loan. Ineligible parents may meet one of the following circumstances: parent(s) are non U.S. citizens or Permanent Residents, have loan bankruptcy and court judgment prevents further debt, and/or parent(s) only form of support is supplemental income, such as SSI, disability, etc.
2021-2022 FAFSA Parental Data Override
This form documents the lack of parental data on the FAFSA in order for a student to qualify for Federal Unsubsidized Stafford Loans. Submit completed form with the required documentation to the Office of Scholarships and Financial Aid (OSFA).
Consortium Agreement Form
The consortium agreement is used to request an increase to your total enrollment at the University of Arizona by including transferable units taken at another university or college. Depending on the types of financial aid you were offered, you may not be able to utilize a Consortium Agreement.
2021-2022 Request For Waiver of Noncustodial Parent Information
Graduate Non-Degree Seeking Federal Financial Aid Petition
Only for pending graduate students currently in non-degree status.
Satisfactory Academic Progress Appeal Form
US Department of Education regulations require students to maintain satisfactory progress towards a degree or certificate. A student who is no longer eligible for federal or institutional financial aid due to a failure to meet satisfactory academic progress standards and who has been placed in Financial Aid Suspension may appeal this status.
General Appeal Form
Students at the University of Arizona have the right to appeal any decision that was made by the Office of Scholarships and Financial Aid (OSFA) by completing and submitting this form to this office. By submitting this form, after exploring all other alternatives, you are asking that your case be reviewed again by a financial aid counselor; however, any decisions made after the appeal are final.
Note, this form should not be used for:
Scholarship Appeals or Deferments
Loss of Income or changes to your and/or your family's financial circumstances. Refer to our appeals page for additional information about the Family Contribution Appeal.
Merit Tuition Scholarship and Arizona Assurance Deferment Request
Used to request a review in order to preserve a semester or academic year use of your tuition scholarship and/or Arizona Assurance eligibility for non-enrollment at the University of Arizona.
Merit Tuition Scholarship and Arizona Assurance Appeal
Used when extenuating circumstances have stopped your Tuition Scholarship and/or Arizona Assurance Funding from being renewed or when you have not been invited or selected for Arizona Assurance but have experienced extenuating circumstances.
Family Contribution Appeal
If a student or their family’s financial situation changes due to special circumstances after filing the FAFSA, a student may request to submit a Family Contribution Appeal. Students must contact our office to request the online form. The student will be required to submit supporting documentation with the Family Contribution Appeal in order to verify the change in circumstances. Note: If you already have an EFC of zero (0), you are not eligible to complete this form.
Cost of Attendance (COA) Appeal
If a student has qualified documented expenses that are incurred during the academic year, which are larger than the standard financial aid cost of attendance, the student may be able to increase their eligibility for financial aid by submitting a COA Appeal. Students must contact our office to request the online form. The student will be required to submit supporting documentation with the COA Appeal in order to verify the expenses. Scholarship Designation Form
Used by scholarship donors to send with checks for specific students.
Health Professions Loan Program Application
This is a supplemental form required by Pharmacy students only for a special loan program. For the 2020-2021 academic year, the Health Professions Student Loan will be awarded to eligible applications in November 2020. Priority deadline to submit application is October 1, 2020 and the application closes January 31, 2021. The 2021-2022 application will be available in July 2021.
Federal Work Study Job Application for OSFA
For students interested in applying for FWS openings in our office.
Authorization for Release of Information
This form allows a student to grant access to a third party, such as a parent not listed on the FAFSA, to receive information regarding their Educational Records. Located under FERPA Forms.
Qualified Tuition Reduction
Available through Human Resources, Qualified Tuition Reduction (QTR) program that enables eligible individuals to enroll in courses of study at reduced registration fees.
Residency classification
Used to obtain resident status for tuition purposes.
Verification of Enrollment Form
This form can be utilized to verify enrollment for a servicer/lender or third party.
In-School Deferment Request
Submit this Request to the Registrar's office, Administration Bldg 2nd Floor.
