Form and Document Submission

Some forms need to be assigned to you by our office. Monitor your To UAccess Student Center To Do List and your CatMail for more information about accessing these forms if they are needed. Other times, you may be required to download a form (listed below) and submit it directly to our office. Check out the instructions below for details on all the convenient ways you can submit your form and/or documentation.

Forms and/or documents can be submitted online via our Ask Aid Portal per the instructions below. Third party organizations can upload documents on behalf of the student. In order to do so, you will need to provide the student’s full name, University of Arizona email, and (8 digit) University of Arizona Student ID number.

  1. Log in with your NetID (or as a guest) and select your student status.
  2. Under “How can OSFA help you today?” select Submit Documents Only.
  3. Complete the Student Information section and, if necessary, the Your Information section.
  4. Attach your file with the Choose File button.
    • Acceptable attachment formats: PDF is preferred or Microsoft Word is also accepted.
  5. Click Submit to send the form to our Ask Aid Team.

The Office of Scholarships and Financial Aid is located in the Administration Building, Room 208.

Special Circumstances

  1. 2024-2025 Unaccompanied Homeless Youth Determination Form 
    Students who are under the age of 24 and who are either currently unaccompanied and experiencing homelessness or self-supporting and at risk of experiencing homelessness may complete this form. This process allows eligible students to be considered as independent students for financial aid purposes. 
  2. Certification of Original Documents
  3. 2024-2025 Parent PLUS Loan Special Circumstances Form
    The Parent PLUS Loan Special Circumstances form is for students whose parents are unable to apply for the Federal Direct Parent PLUS Loan. Ineligible parents may meet one of the following circumstances: parent(s) are non U.S. citizens or Permanent Residents, have loan bankruptcy and court judgment prevents further debt, and/or parent(s) only form of support is supplemental income, such as SSI, disability, etc.
  4. Parental Data Override Form
    This form documents the lack of parental data on the FAFSA in order for a student to qualify for Federal Unsubsidized Stafford Loans. Submit completed form with the required documentation to the Office of Scholarships and Financial Aid (OSFA).

  5. Spring 2025 Consortium Agreement Form
    The consortium agreement is used to request an increase to your total enrollment at the University of Arizona by including transferable units taken at another university or college. Depending on the types of financial aid you were offered, you may not be able to utilize a Consortium Agreement.
    Submission Deadline: December 13, 2024
  6. 2024-2025 Request for Waiver of Non-FAFSA Parent Information
    This form can be used by students who are under consideration for Arizona Assurance or Achievement Award and are required to have their Non-FAFSA Parent submit the Resource Evaluation. This form should be submitted only if unique circumstances exist which would prevent the Non-FAFSA Parent from completing the Resource Evaluation Form. If approved, the Non-FAFSA Parent Resource Evaluation requirement will be waived.
    The deadline to submit this form has passed.
  7. Graduate Non-Degree Seeking Federal Financial Aid Petition
    Only for pending graduate students currently in non-degree status.

Appeals

  1. Satisfactory Academic Progress Appeal Form
    US Department of Education regulations require students to maintain satisfactory progress towards a degree or certificate. A student who is no longer eligible for federal or institutional financial aid due to a failure to meet satisfactory academic progress standards and who has been placed in Financial Aid Suspension may appeal this status. Visit the SAP Policy page for more information.
  2. General Appeal Form
    Note, this form should not be used for:
    • Scholarship Appeals or Deferments
    • Loss of Income or changes to your and/or your family's financial circumstances. Refer to our appeals page for additional information about the Family Contribution Appeal.
  3. Merit Tuition Scholarship and Arizona Assurance Deferment Request
    Used to request a review in order to preserve a semester or academic year use of your tuition scholarship and/or Arizona Assurance eligibility for non-enrollment at the University of Arizona.
    The Spring 2025 submission deadline is January 15, 2025.
  4. Merit Tuition Scholarship and Arizona Assurance Appeal
    Used when extenuating circumstances have stopped your Tuition Scholarship and/or Arizona Assurance Funding from being renewed or when you have not been invited or selected for Arizona Assurance but have experienced extenuating circumstances.
    The 2024-2025 submission deadline has passed.
  5. FAFSA Data Appeal
    If a student or their family’s financial situation changes due to special circumstances after filing the FAFSA, a student may request to submit a Family Contribution Appeal. Students must contact our office to request the online form. The student will be required to submit supporting documentation with the Family Contribution Appeal in order to verify the change in circumstances.     Note: If you already have an SAI of -1500, you are not eligible to complete this form.
  6. Cost of Attendance (COA) Appeal
    If a student has qualified documented expenses that are incurred during the academic year, which are larger than the standard financial aid cost of attendance, the student may be able to increase their eligibility for financial aid by submitting a COA Appeal. Students must contact our office to request the online form. The student will be required to submit supporting documentation with the COA Appeal in order to verify the expenses. Other Forms

  1. Scholarship Designation Form
    Used by scholarship donors to send with checks for specific students.
  2. Health Professions Loan Program Application
    This is a supplemental form required by Pharmacy students only for a special loan program.  The timeline for the 2024-2025 application cycle is as follows: 
    1. Reviews and awarding will begin in mid-November, 2024. 
    2. The application deadline is January 31, 2025. 
  3. Authorization for Release of Information
    This form allows a student to grant access to a third party, such as a parent not listed on the FAFSA, to receive information regarding their Educational Records. Located under FERPA Forms.
  4. Qualified Tuition Reduction
    Available through Human Resources, Qualified Tuition Reduction (QTR) program that enables eligible individuals to enroll in courses of study at reduced registration fees.
  5. Residency classification
    Used to obtain resident status for tuition purposes.
  6. Verification of Enrollment Form
  7. Right to Cancel or Return Loan Funds
    This form can be utilized to verify enrollment for a servicer/lender or third party.
  8. In-School Deferment Request
    Submit this Request to the Registrar's office, Administration Bldg 2nd Floor.
