What documentation is sufficient to prove my Citizenship?

There are 2 ways you can complete the Citizenship Verification requirement. You may either submit your documentation online or you may submit your documentation in-person at the Office of Scholarships and Financial Aid.

1. To confirm your citizenship status online: Review the instructions in your UAccess Student Center to navigate to the Confirmation Your Citizenship Status online form. 

Use this form to submit two different types of documentation . You will need to provide:

  • 1 copy of a valid photo ID (Examples: State issued Driver's License or Identification Card, etc.)

AND

  • 1 copy of your Citizenship and/or Immigration Document (Example: Permanent Resident card, I-94 Form, etc.)

*Please note that you can not use your passport twice for your proof of citizenship and your government issued Photo ID. Unfortunately we can not accept military IDs as documentation.

2. To confirm your citizenship status in-person: Visit the Office of Scholarships and Financial Aid and provide the following documents for review:

  • 1 copy of a valid photo ID (Examples: State issued Driver's License or Identification Card, etc.)

AND

  • 1 copy of your Citizenship and/or Immigration Document (Example: Permanent Resident card, I-94 Form, etc.)

Examples of documents that can be presented to confirm your citizenship status:

  • U.S. Birth Certificate
  • U.S. Passport or Passport Card (signed)
  • U.S. Travel Document
  • U.S. Certificate of Naturalization (signed)
  • U.S. Certificate of Citizenship (signed) Certificate of Birth Abroad
