What are the different ways I can complete the Statement of Educational Purpose that has been requested?
Answer
You can either complete the State of Educational Purpose requirement for verification in-person or by mailing an original copy of the completed form to our office
To appear in person at our office, please:
- Print the Statement of Educational Purpose Form
- Appear in person at the University of Arizona Office of Scholarships and Financial Aid (https://financialaid.arizona.edu/contact) and present a copy of your photo ID to verify your identity
- Please bring any unexpired form of government issued photo identification such as a driver license, passport or other state-issued ID. Please note that we are unable to accept Military IDs as we can not make a copy for our records.
- Complete to top portion of the Statement of Educational Purpose in front of a Financial Aid Administrator
If you are unable to appear in person at our office, you can mail an original copy or our office:
- Print the Statement of Educational Purpose Form
- Bring the form to a notary public and present a copy of your photo ID to verify your identity. You must visit a notary public in person, as we can not accept the use of an online notary public.
- Complete to top portion of the Statement of Educational Purpose in front of a notary public
- Make a copy of the unexpired valid government issued photo ID that is presented to the notary public.
- Examples are a driver license, passport or other state-issued ID. Please note that Military IDs are not accepted due to our limitation on making a copy.
- Mail both the completed form and a copy of your photo ID used to:
